• 2010 Nominations Ballot. If you are a member (paid dues since January 2010 – as in you paid in January, February, or March 2010), and you will not be attending the Social on April 1, 2010, then print out the ballot and fax it to Jerry Ankiewicz at Adultcare Assistance Homecare: 623-977-8764, BEFORE April 1, 2010. It will not be counted if you send it in AFTER April 1, 2010.  If you have any questions, please call Jerry at 623-977-2223. Click HERE for ballot.
